Provider Network Criteria

Thank you for contacting Communities Connected for Kids, Inc. to learn more about joining our network of providers. At the present time we are not actively seeking to contract with additional group homes in our area due to the complexities of changing legislation with regard to residential programs. If you are still interested in opening a group home, please visit http://centerforchildwelfare.fmhi.usf.edu/FFPSA.shtml to learn more about how recent legislation will impact funding and programming.

However, if you are not a group home provider, we continue to welcome proposals from our community. Proposals should include items such as:

  • Executive Summary & Description of Organization
  • Federal Employment Identification Number
  • Supporting documentation of status as for-profit or not-for-profit
  • Accreditation status
  • Organizational Chart
  • List of Oversight Board Members (if applicable)
  • Specific proposed program capacity, design & description
  • Demographics of customers/clients to be served in program (i.e. boys, ages 12-18)
  • Potential/current location of intended services of program
  • Proposed implementation plan with timeline prospectus for program
  • Outline of planned program staffing pattern – professional qualifications, ratio information, prior experience, etc.
  • Policies & Procedures – inclusive of such items as: child handbook, rules & regulations, emergency preparedness plan, quality improvement plan, progressive disciplinary practices (personnel & client), plan for record-keeping, financial capacity & segregation of duties, etc.
  • Goals and specific planned program performance outcomes/objectives
  • Most recent financial audit/monitoring report – Review of Business
  • Experience & demonstrated performance:  Principals of organization and their associates prior business experience in specific capacity of program proposal
  • Line item budget with narrative (along with cross-correlation to requested rate)
  • 2 Business references & 1 personal reference for the lead CEO/Executive Director (and other principles)
  • Current/plan for insurance
  • Current/plan for DCF state licensure or certification

At times our organization identifies a specific program or service that is needed within our network. At such time, we may refer to proposals received and contact associated soliciting organizations to advise them that a competitive procurement process has been initiated. The Family First Prevention Services Act (FFPSA) was passed and signed into law on February 9, 2018 (visit http://centerforchildwelfare.fmhi.usf.edu/FFPSA.shtml to learn more). Among the provisions of this new law, a clearinghouse of approved intervention and prevention services was developed. At this time, CCKids will only consider parenting programs that are included in the approved clearinghouse. Visit https://preventionservices.abtsites.com/program for the list of approved programs.
